Secondary transition planning or programming has been shown to be beneficial to a student’s post-school success. Because transition planning is an ongoing, multi-component process, school personnel need a way to ensure that they are implementing it effectively for students with disabilities. One framework commonly used for this purpose is the Taxonomy for Transition Programming, as developed by the researcher Paula D. Kohler (2016). This framework helps practitioners to organize five critical components of secondary transition planning:

- Student-focused planning
- Student development
- Family engagement
- Interagency collaboration
Because they are generally the most involved with the student, teachers are instrumental in the transition planning process and need to understand how they can participate in the implementation of these five components. We will take a closer look at each of them on the subsequent pages.
